If you change your name and/or address, and you have a South Australian driver's licence or a vehicle registered in South Australia, you must advise the department within 14 days or penalties may apply. You can:

• submit a change of address online
• phone the department on 13 10 84 (8 am - 6 pm, Monday to Wednesday and 8 am - 8 pm, Thursday to Friday)
• send written notification to a Customer Service Centre

or

• complete an 'Application for Change of Address' form at any Customer Service Centre. You will be required to provide evidence of identity eg driver's licence to change your address in person.

If you are notifying your change of address by phone or over the Internet you must provide your:

• client number (from your driver's licence or registration papers)
• name
• date of birth
• previous address
• new address.

If you have changed your name you must attend a Customer Service Centre in person and will be required to provide evidence of identity and proof of change of name. Please call 13 10 84 for details.
